Additional Fees & Information: *95.00 per retouch image. Any additional individual in the image will be $55.00 per person.

Please arrive on time for your session. If you are running late, we would appreciate a call to help us hold your scheduled time. After a 15-minute grace period, your appointment will be automatically canceled and rescheduled for a later date.

Please note that deposits are non-refundable and non-transferable.

Prior to booking your appointment, please fill out and submit the Waiver of Liability Relating Coronavirus/COVID-19. (Required for photo session)

Once submitted you will be directed to our appointment page. 

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

Jackie Hicks Portrait Artist “Jackie Hicks” dba Fond Memories has put preventative measures in place to reduce the potential spread of COVID-19; however, Fond Memories cannot guarantee that the virus will not be transmitted to you and/or your child(ren) causing you to become infected with COVID-19. Further, Fond Memories has taken all the necessary steps to clean and disinfect the Fond Memories studio. It will be sanitized after each client by using disinfectant and cleaning agents recommended by health care professionals.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I, my spouse child(ren), and/or any agents that accompany me to Fond Memories may be exposed to or infected by COVID-19 as a result of utilizing our Portrait and Makeup Service at Fond Memories and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that there is a risk of becoming exposed to or infected by COVID-19 and that I should come prepared to have my temperature checked.
 

I will be provided with appropriate protective equipment including but not limited to shoe covers, a disposable cape, and if needed gloves. The Photographer and Make-up Artist will be in full protective gear: Masks, Face Shields, Disposable Capes, shoe covers, and gloves. There will be no large parties or groups permitted in Fond Memories.

Once you have arrived at the studio please call Jackie Hicks at 301-254-3954 to notify her of your arrival. You must do this so that she can instruct you on how to enter the studio for your photo session.
